This form is ONLY for “Safe Start” or Health Officer Order violations observed in Businesses. We encourage any individual to also report Safe Start or Health Officer order violations to Washington State.
FOR INDIVIDUAL OR PRIVATE GROUP SUSPECTED VIOLATIONS: Consistent with the Governor’s Safe Start order, you may report any suspected violations by individuals or private groups to your local law enforcement agency. DO NOT call 911. Please refer to the law enforcement telephone number listed on the complaint form.
Governor Jay Inslee, the Washington State Department of Health, Jefferson County's Health Officer Tom Locke, and the Dept. of Labor & Industries have issued orders that require businesses to adopt appropriate health and safety measures for staff and customers and require individuals to wear a face covering when in most public spaces. If this report is about a business that is not following the COVID-19 mandated requirements fill out the form found in the link below. This report will be forwarded to the appropriate agency to review the suspected violation for the appropriate response.
