Jefferson County Public Health Offers Sports Physicals at School Based Health Clinics
Port Townsend – Get ready for fall sports!
Jefferson County Public Health offers Sports Physicals for any student athlete. Make an appointment for your sports physical with the School Based Health Center in Port Townsend High School or Chimacum High School. The School Based Health Centers are a partnership between Jefferson County Public Health, Jefferson Healthcare, and the local school districts, providing health care services to meet the needs of adolescents. Teen Vaccinations are also available!
Chimacum High School: August 15, 17, 22
Port Townsend High School: August 14, 16, 21
Appointments are available 9am to 3pm
Open to any student who needs sports physical
We can bill your insurance or we can perform your physical for $45.
